Before your first session, please complete the following forms. If you are unable to complete the forms before your session, I will be happy to complete them with you during our first session.

  • Professional disclosure statement

 

The Release of Information (ROI) is only needed if there are other providers you would like me to collaborate with (i.e. physician, psychiatrist, teacher, family member).

*If your child is under the age of 18, they are considered a minor and will require parent(s) or legal guardian signature(s) on all forms.
